We help to keep our community healthy. Homewood provides essential mental health and addiction services to residents of Guelph and Wellington County. From education and outreach to outpatient programming to crisis care, we help individuals function optimally in our community.
Trillium Acute Care Program for Crisis Support
Guelph-Wellington residents manifesting suicidal behaviour or severe disruption of self-care, may be assessed and stabilized in this program.
Assertive Community Treatment Team (ACTT)
Local residents receive help to achieve their goals, improve their health and quality of life, and live as independently as possible.
Community Addiction Services (CADS)
Addiction programs for Guelph-Wellington residents are provided on an outpatient basis through Homewood Community Addiction Services.
Program for Older Adults (POA)
The Program for Older Adults’ multidisciplinary team offers assessment, treatment and recommendations for the care of older adults.
Comprehensive Psychiatric Care (CPC)
The care provided in this inpatient program aims to minimize chronic illness symptoms and help individuals to reach their maximum level of functioning.
Emergency Mental Health Service (EMHS)
Guelph-Wellington County residents may be admitted to this unit located at Guelph General Hospital for crisis-related mental health care.
Support Coordination
Recovery for people with serious and persistent mental health challenges and/or concurrent addiction concerns
Transitional Care Program
People who struggle with complex mental health and co-occurring needs often require support to function successfully in the community.
Seniors Support
Support for older adults with cognitive impairment who may be transitioning in their living arrangements, or moving from a hospital into the community.
